The United States Department of State has made it easier to access DS-156 and other visa application forms by making the application forms available on the Internet. The following outlines the process by which one can locate the OS-156 application online, fill it out, and print the application before appearing at the Embassy for the visa Interview. Allow approximately 20 minutes to complete and print the form. It is as easy as 1-2-3.

1. First, locate the D5-156 form online at http://evisaforms.state.gov. Scroll down to the bottom of this home page and click on the “DS-156’ form.
2. 2. Second, fill out the form on the computer completely. After completing the application, click the “Continue” button at the bottom of the form.
3. 3. Third, print the form. Be sure to submit all three (3) pages printed at the time of the interview.

Some applicants will also need to complete forms DS-157 and/or DS-158. They are available at the following websites:

These forms are not active web sites as that for the DS-156. However, applicants may fill out the forms on screen and print.
